1) Will be positive, supportive and ethical in my actions to each and every person (players, coaches, other spectators) on my team as well all individuals on the opposing team.

2) Understand that there is no guaranteed playing time and accept that the coaches, and only the coaches, determine positions and playing times.

3) Parents must respect the 24 hour rule: if communication with team coach is necessary, please wait 24 hours after competition to initiate communication so that emotions are reduced and communication can be more productive.

4) Will not attempt to “coach” players from the sidelines, because players must listen to their coaches and their instructions.

5) Will learn the rules of the game to better understand decisions of the coaches and game officials.

6) Will appreciate the time and energies donated by the team manager and team representatives, and cooperate with their requests.

7) Will follow the communication guidelines set out by the Club, that is:

   1: Manager or coach of Team

   2: Area/Division Director

   3: Program Director

   4: Boys or Girls Director of Coaching

   5: Vice President of Youth Soccer

8) Will set a positive example for the players by respecting game officials and accepting their decisions, whether they are agreed with or not. Encourage fair play, good sportsmanship, competitive play, and cheering instead of yelling. (Aparent acting-out in a foul or abusive manner during any game can also be ejected from the field with a red card resulting in a sendoff penalty against the team as well as a personal suspension and a possible fine pending a hearing from their respective League A&D Committee).

9) Will refrain from: questioning, criticizing or abusing officials, taunting or booing players or officials, using profanity or degrading gestures, coaching from the sidelines.

10) Will pay my child’s dues on time to ensure he/she is eligible to play.

11) Understand that Texas law requires me to report any suspicion of abuse or neglect of a child, and that FC Dallas and its staff are bound by the Safe Sport Act with regard to incidents of potential abuse. If you have any questions about this, please contact childsafety@fcdallas.com.

12) Will adheare to all club protocols related to public health and safety (including social distancing, masks, limitations on access due to COVID-19 as directed by club).

NOTE: In competitive soccer, if any “non-player”, i.e. parent, friend, etc. is asked to leave the sidelines by the referee due to misbehavior, verbal or otherwise, and does not comply the COACH may then be instructed to leave – leaving the team without a coach during the game.

FC Dallas Youth Player Code of Conduct

1) I will conduct myself, at all times, in a professional and sportsmanlike manner, by treating my teammates, competitors, officials, and all others involved with courtesy and respect.

2) I will, at all times, use language that is appropriate. I understand that the use of obscene language or gestures, curse words, racial slurs, gender slurs, or sexually abusive insults will not be tolerated at any time.

3) I will abide by all applicable facility and tournament rules at all times and will act in a manner that is respectful of Club event and its facilities. Players will show respect for the event facilities by taking care in their actions, such as by removing trash or debris from around the bench area after each training session or game.

4) Will adhere to all club protocols related to public health and safety (including social distancing, masks, limitations on access due to COVID-19 as directed by club).

5) I will not tolerate, encourage, aid, assist, or participate in hazing of any form, whether directly or indirectly. Players must understand that hazing includes any act or situation that inflicts (or threatens to inflict) physical, emotional, or psychological pressure or injury on an individual or that purposely demeans, degrades, or disgraces an individual. Players must understand that all FC Dallas Youth players and other persons affiliated with FC Dallas Youth are subject to these same prohibitions against hazing. If a player witnesses, becomes aware of, or has information relating to any hazing incident, the player must report it to the head coach as soon as possible and cooperate with any investigation regarding the incident.

I agree to abide by the code of conduct. I understand that I play an important role in creating a positive environment for player development. Furthermore, I understand that I represent the FC Dallas professional organization in the community and as such my conduct will be appropriate and supportive of the guidelines stated above. Failure to comply with the above may result in disciplinary action including but not limited to a fine. If a fine is imposed and is not timely paid by the due date, further sanctions may result including suspension from the team or club. Removal from the club will not alleviate any outstanding financial obligations owed to FC Dallas Youth.

In consideration of the services provided by FC Dallas Youth LLC (“FCDY”), the undersigned hereby promises to pay the following fees and administrative costs covering participation in FCDY programs during the 2022-2023 season which ends June 30, 2023 (“the Fees”).  The Fees shall be paid in accordance with the online payment plan election which corresponds to the signed Fee Agreement made by the undersigned. Only one selection may be made.

The Fees may be prepaid at any time, in whole or in part, without penalty, however, the administrative fees shall still apply. All payments are payable to FC Dallas Youth LLC and as such must be sent directly to the club when payment is remitted.  There are no refunds, credits or reductions to the payment plan; and payments must be received by FCDY no later than the agreed upon due date.

FCDY follows a "NO PAY, NO PLAY" policy; which means that a failure to pay as agreed herein may result in the player whose name is listed below (the “Player”) not being permitted to play. The Player will not be allowed to participate with his/her club team until such time that their account is brought current, including any late fees assessed by FCDY at its discretion.  The Player’s coach (the “Coach”) will have their pay docked the pro rata share of the fees attributable to any unpaid fees with respect to the Player. FCDY reserves the right to deny a release or transfer. All Players are required to fulfill their financial obligations to FCDY regardless of any circumstance.

Failure to make any payment due hereunder by the due date stated above shall also be a default entitling FCDY to demand full payment of the outstanding portion of the Fees by providing written notice of such demand and allowing the undersigned ten calendar days to make such full payment.  In the event, following notice as provided hereunder, the undersigned fails to make such payment, FCDY, or the Coach may place the debt with an attorney or agent for collection, in which event the undersigned agrees to pay all reasonable attorney fees and costs of collection.  All payments hereunder shall be made to FC Dallas Youth LLC, 9200 World Cup Way Suite 202, Frisco, TX 75033, or any other such address as may from time to time be designated by FCDY, or an attorney or debt collection agency acting on its behalf.

The policies and procedures of FC Dallas Youth, LLC are subject to change at its discretion. FC Dallas Youth will provide written copies of any amendments to these policies and procedures prior to their becoming effective. The undersigned agrees to comply with such amendments and execution of this document makes such amendments binding unless FC Dallas Youth receives written objection. This agreement shall be enforced in accordance with the laws of the state of Texas.

By signing this Acknowledgment and Assumption of Risk, I, the undersigned:

1. acknowledge the contagious nature of COVID-19 and all related forms of the novel coronavirus including any mutations in existence or that come into existence during the Term (“COVID-19”) and voluntarily assume the risk that my child(ren) and/or myself may be exposed to or infected by COVID-19 by participating in any activity outside of our home, including activities organized by FCDY at Toyota Soccer Center, FC Dallas Performance Center, MoneyGram Soccer Park, or other facility authorized by FCDY to host such activities (the “Activities”), and that such exposure or infection may result in personal injury, illness, permanent disability, and/or death;

2. understand that children participating in youth sporting events or practice should minimize in-
person contact with any person 65 years of age or older, especially those with pre-existing
health conditions, for a period of 14 days following the Activities. This includes maintaining
social distancing of at least 6 feet of separation from those individuals, wearing a face covering
or mask, and avoiding sharing utensils or other common objects with those individuals.

3. understand that the risk of becoming exposed to or infected by COVID-19 may be increased by
participation in the Activities, and that such exposure or infection may result from the actions,
omissions, or negligence of myself and others, including, but not limited to, FCDY employees,
coaches, volunteers, and other participants and/or their families;

4. acknowledge that I received and read the materials provided by FCDY setting forth the protocols
that must be adhered to by all participants and their parents/guardians/chaperones in
connection with the Activities until further notice;

5. understand that these protocols are necessary for the health and safety of all participants in the
Activities;

6. take full responsibility for both mine and my children’s adherence to all such protocols including
self-screening, if required, prior to each and every participation in the Activities;

7. understand that my participation in the Activities is contingent on my acceptance of all the
terms herein and that I must provide a copy of the confirmation of such acceptance to my
child’s coach or team manager before they will be allowed to participate; and

8. understand that these protocols may change at any time.

In exchange for the license and privilege of access to and/or participation in this event or activity, the undersigned hereby forever releases, holds harmless, waives, discharges, and indemnifies the FC Dallas Soccer Club™, FC Dallas Soccer, LLC, Frisco Stadium, LLC, FCD Indoor Training, LLC, Major League Soccer, L.L.C., FCD Youth, LLC, FC Dallas Foundation, and their respective affiliates, officers, directors, agents, representatives, sponsors, shareholders, partners, employees, and members (collectively, the “FCD Parties”) from and against any and all liabilities, debts, obligations, costs, expenses, damages, judgments, liens, claims, and demands of any nature or description, in equity or at law, that the undersigned (or his or her heirs or personal representatives) may now have, acquire, own, or hold, or at any time heretofore owned or held, or could or may hereafter own, acquire, or hold against any one or more FCD Parties, known or unknown (including those risks associated with COVID-19 or subsequent public health emergencies), suspected or unsuspected, asserted or not, arising out of participation by the undersigned in events or activities conducted by any one or more of the FCD Parties even if the risks and liabilities that the undersigned is releasing arise out of the negligence or carelessness of one or more of the FCD Parties (collectively, the “Released Claims”). This release is specifically intended to be binding on all of the undersigned’s heirs and assigns. THE UNDERSIGNED ADMITS THE POSSIBILITY THAT THE UNDERSIGNED OR HIS OR HER SUCCESSORS MAY NOT FULLY KNOW THE NUMBER OR MAGNITUDE OF ALL RELEASED CLAIMS, BUT NEVERTHELESS INTENDS TO ASSUME ALL RISKS BY RELEASING SUCH UNKNOWN CLAIMS AND AGREES THAT THIS RELEASE IS A FULL AND FINAL RELEASE AND WAIVER OF ALL RELEASED CLAIMS. THE UNDERSIGNED EXPRESSLY WAIVES THE BENEFIT OF ANY STATUTORY OR OTHER PROVISION THAT CREATES ANY EXCEPTION TO THE FOREGOING GENERAL RELEASE, WAIVER, AND AUTHORIZATION.

The undersigned further grants to the FCD Parties, jointly, for their own benefit and for the benefit of others, the non-exclusive and perpetual right to (a) interview, photograph, or otherwise capture the image of the undersigned, both in still and motion pictures, (b) record the voice of the undersigned, and (c) use and duplicate such photographs or other images, whether in print, videotape, digital, or other formats, and reproduce the voice of the undersigned, for all purposes. All copies of the undersigned’s image, likeness, and voice created or recorded by the FCD Parties (or their agents) hereunder shall be the exclusive property of the FCD Parties, jointly. The undersigned agrees that the copyright in any work captured by the FCD Parties (or their agents) hereunder shall be the exclusive property of the FCD Parties with all such rights in and to said work having been transferred by the undersigned to the FCD Parties hereby.

The undersigned has read this document, understands the contents, has had the opportunity to obtain legal advice as to its effect, and is under no duress or obligation of any kind to execute it. This General Release and Waiver reflects the understanding of the undersigned with respect to all subject matter addressed herein and will be construed accordingly.

THE UNDERSIGNED UNDERSTANDS THAT THESE ACTIVITIES ARE INHERENTLY RISKY AND DANGEROUS AND AGREES TO ASSUME ALL RISKS AND DANGERS ASSOCIATED WITH PARTICIPATION IN THESE ACTIVITIES.

The undersigned authorizes FC Dallas Youth, LLC to release, upon written request, certain information about the participant to FCDY healthcare sponsors for the limited purpose of monitoring the use of healthcare services by participants of FCDY programs.  Such disclosures will include only the name and date of birth of the participant, may be used only in compliance with HIPAA and other applicable laws and regulations, and will be designated as confidential information.  This consent may be revoked at any time by the undersigned by written notice to FCDY.  Any such revocation will not apply to information that has already been released.  A photocopy of this consent shall be considered as valid as the original.

Every FCDY player must order the full required uniform kit from soccer.com and are not permitted to purchase their uniforms from any other provider. No other provider other than soccer.com stocks the official FCDY uniforms (though they may tell you they do). In the event of a club mandated change in uniform, the club will update any uniform purchased from soccer.com at no charge to the player. These updates will be at the full cost to players wearing unofficial uniforms. Player may be sanctioned (including loss of playing time) for failure to abide by this provision. Each player will receive a customized email to order their uniform through soccer.com. By signing below, I understand and agree to purchase the full mandatory uniform kit via www.soccer.com. The cost of the uniform is not included in the player’s club dues payment plan.
